Coaching Services Disclaimer and Agreement
Welcome!
Before we begin, I would like to share some important information to ensure we are clear, aligned, and comfortable moving forward.
Although I am a physician by training, I no longer practice medicine in any capacity. In this work, I serve only as a Holistic Life Coach. I provide support, guidance, and tools for personal growth and well-being.This means:
- What I Do:Together, we explore practical ways to step out of stress and overwhelm, creating more space for lightness, balance, and ease.
- What I Don’t Do: Coaching is not medical care or psychotherapy. I do not diagnose, treat, or prescribe. If you are facing medical or psychological challenges, I encourage you to work with licensed healthcare providers. Coaching is a complement, not a replacement.
- In Case of Emergency: If you feel unsafe, overwhelmed, or at risk of harming yourself or others, you agree to call 911 (or your local emergency number), go to the nearest emergency room, or reach out to a trusted person for immediate help. I do not provide crisis services, and I do not monitor messages in real time. My role is to walk alongside you once you are safe.
- Your Responsibility: You are the expert in your own life. I offer perspective and reflection, but all choices and outcomes are yours to make. While I cannot guarantee results, I believe in your capacity for growth and gentle shifts toward steadier well-being.
To ensure we begin with complete clarity, I have outlined the formal agreements for all coaching sessions, workshops, retreats, and materials I offer below.
1. Scope of Services
- I offer holistic life coaching to support growth, self-discovery, and overall wellness.
- I do not practice medicine, psychotherapy, psychiatry, or any other state-licensed healthcare profession in this role.
- Our work together is educational and supportive in nature, and it does not constitute medical advice.
2. No Medical or Mental Health Care No Physician‑Patient Relationship
- Coaching does not diagnose, treat, cure, or prevent any medical or psychological condition.
- I do not prescribe, adjust, or recommend medications.
- Our coaching relationship is not a physician-patient relationship and is not telemedicine.
- If you have medical or psychological concerns, you agree to seek prompt evaluation and care from a licensed healthcare provider. You agree not to delay, avoid, or stop medical or mental health treatment based on coaching.
3. Emergency and Crisis Situations
- Coaching is not appropriate for emergency or crisis situations.
- If you are suicidal, considering harming yourself or others, or in crisis, you agree to immediately call 911 (or your local emergency number) or go to the nearest emergency facility.
- You understand that I do not provide crisis intervention, emergency services, or on-call coverage.
4. Personal Responsibility and Assumption of Risk
- You are solely responsible for your own physical, mental, and emotional health, your decisions, and your actions at all times.
- No results are promised or guaranteed. Outcomes vary for each person.
- You understand that coaching can bring up emotions, memories, and insights. You agree to seek additional support, including medical or mental health care, if needed.
- You voluntarily choose to participate in coaching, and you assume full responsibility for any risks, known or unknown, that may arise from your participation.
5. Confidentiality and Its Limits
- Your coaching sessions are held in confidence. I will not share information about you without your permission, except as described in this policy or in my Office Policies and Client Consent.
- There are legal and ethical limits to confidentiality. I may need to disclose information if:
- I reasonably believe you pose a danger to yourself or others.
- There is suspected abuse or neglect of a child, elder, or vulnerable adult.
- I am required to comply with a court order, subpoena, or other legal process.
- For full details about confidentiality, record-keeping, and communication, please review and sign my Office Policies and Client Consent form.
6. Fees, Scheduling, and Cancellations
- Information about fees, payment, scheduling, and cancellations is described in my Office Policies and Client Consent, which you will receive separately.
- By signing this form, you acknowledge that you have received or will receive those policies and that they are part of our overall agreement.
7. Intellectual Property
- All materials I provide, including written content, recordings, exercises, and tools, are for your personal use only.
- You agree not to copy, distribute, sell, or publicly share these materials without my written permission.
8. Limitation of Liability and Release
- To the fullest extent permitted by law, you agree to release and hold harmless Jessica Wei, MD, from any and all liability, claims, or damages arising directly or indirectly from:
- Your participation in coaching services, workshops, or retreats.
- Your use or misuse of any information, suggestions, or materials provided.
- This includes, without limitation, any physical, mental, emotional, or financial outcomes that may occur.
- In no event will I be liable for any indirect, incidental, special, or consequential damages that result from your participation in coaching.
9. Confidentiality and Its Limits
- By signing this Agreement, you confirm that you are at least 18 years old and legally able to consent to coaching services.
10. Governing Law
- This Agreement is governed by the laws of the State of Massachusetts, without regard to its conflict of law rules.
- If any part of this Agreement is found to be invalid or unenforceable, the remaining provisions will remain in full force and effect.
11. Purpose of Services
- Coaching with me is intended to support personal growth and gentle shifts toward steadier well-being.
- It is not a substitute for licensed medical or psychological care.
